Early Registration Rate
To receive the special early registration rate, register by April 2, 2021.

Mail and Faxed Payment Options:

• Mail checks payable to: MNSHA. Postmarked by April 2, 2021 to receive the early registration rate.
• Credit card services are available. (Visa, MasterCard, Discover and AMEX).
• Purchase Order (attach with registration form) – Registrations not accepted without approved purchase order forms.

Pre-registration is strongly encouraged as availability is not guaranteed with on-site registration.

Printable Registration Form

Group Registration is available when people from the same workplace register to attend Convention together. Discounts are available based on the number of attendees.

• Printable Group Registration Form→ Employer must complete this form and include it with all individual Printable Registration Forms. All documents can be mailed or faxed with a check or credit card for payment.
• Online Group Registration Form→ Complete this online form and include scanned registration forms. This online version is available with credit card payment only.

Cancellations and Refunds

Full refunds (minus a $25 handling fee) will be made only if written cancellation request is received no later than April 2, 2021.

All cancellations and refunds will be processed after the Convention.
